Key Responsibilities
- Conduct comprehensive virtual patient assessments, including history taking, physical exams (as appropriate for telehealth), and diagnostic interpretation.
- Diagnose and treat a wide range of acute and chronic conditions within the scope of practice for a Nurse Practitioner.
- Develop and implement individualized treatment plans, including medication prescribing, lifestyle modifications, and referrals to specialists when necessary.
- Provide thorough patient education on diagnoses, treatment options, preventive care, and health management.
- Maintain accurate and timely documentation of all patient encounters in the Electronic Health Record (EHR) system.
- Collaborate effectively with interdisciplinary healthcare teams, including physicians, specialists, and care coordinators, to ensure integrated patient care.

Required Skills
- Current, unrestricted Nurse Practitioner (NP) license in the state of Illinois.
- Board certification as a Nurse Practitioner (e.g., FNP, AGPCNP, PMHNP).
- Minimum of 3 years of clinical experience as a Nurse Practitioner.
- Demonstrated experience or strong familiarity with telehealth platforms and virtual patient care delivery.
- Excellent clinical assessment, diagnostic, and treatment planning skills.
- Superior verbal and written communication abilities, with a compassionate and empathetic approach.
- Proficiency in using Electronic Health Records (EHR) systems.
