Key Responsibilities
- Conduct comprehensive virtual patient assessments, including history taking, physical exams (via telehealth technology), and diagnostic ordering.
- Diagnose and treat common acute and chronic illnesses, injuries, and health conditions in accordance with established protocols and best practices.
- Develop and implement individualized care plans, including medication management, lifestyle modifications, and referrals to specialists when appropriate.
- Provide patient education on disease prevention, health promotion, medication adherence, and self-management strategies.
- Accurately document all patient encounters, assessments, diagnoses, treatments, and care plans in the electronic health record (EHR) system.
- Collaborate with physicians, other nurse practitioners, and allied health professionals to ensure coordinated and integrated patient care.
- Maintain strict adherence to privacy regulations (HIPAA) and all relevant clinical guidelines and professional standards.
- Participate in quality improvement initiatives and ongoing professional development to enhance clinical skills and telehealth practices.
- Provide empathetic and culturally competent care to a diverse patient population.
Required Skills
- Active, unrestricted Nurse Practitioner license in Pennsylvania (or multi-state compact license with PA privileges).
- Master's or Doctoral degree in Nursing from an accredited program.
- Board Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or equivalent.
- Minimum of 3 years of clinical experience as a Nurse Practitioner, preferably in primary care, urgent care, or a telehealth setting.
- Proficiency with electronic health records (EHR) systems and telehealth platforms.
- Excellent verbal and written communication skills, with the ability to build rapport with patients virtually.
- Strong critical thinking, problem-solving, and diagnostic abilities.
- Ability to work autonomously and manage a diverse patient caseload effectively.
- Comfortable with technology and capable of troubleshooting minor technical issues independently.
Preferred Qualifications
- Experience with specific telehealth software platforms.
- Bilingual proficiency (e.g., Spanish) to serve a diverse patient population.
- Additional certifications in specialty areas relevant to primary care or chronic disease management.
