Key Responsibilities
- Conduct comprehensive virtual health assessments, including history taking and physical exams via telehealth platforms.
- Diagnose and treat acute and chronic illnesses within the scope of practice, utilizing evidence-based guidelines.
- Develop and implement individualized patient care plans, including medication management and lifestyle recommendations.
- Order and interpret diagnostic tests, and refer patients to specialists as needed.
- Provide patient education on health conditions, preventive care, and medication adherence.
- Maintain accurate, timely, and thorough electronic health records (EHRs).
- Collaborate effectively with a multidisciplinary healthcare team to ensure coordinated patient care.
- Adhere to all legal, ethical, and professional standards of practice, including HIPAA compliance.
- Participate in ongoing professional development and continuing education relevant to telehealth and primary care.
- Utilize telehealth technology proficiently for secure and effective patient interactions.
Required Skills
- Current, unencumbered Nurse Practitioner (NP) license in Georgia (or eligible for licensure).
- Master's or Doctoral degree from an accredited Nurse Practitioner program.
- Board Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or similar.
- Minimum of 3 years of clinical experience as a Nurse Practitioner.
- Proficiency in diagnosing and treating a broad range of common medical conditions.
- Excellent verbal and written communication skills with strong interpersonal abilities.
- Demonstrated empathy, compassion, and patient-centered approach to care.
- Strong critical thinking and problem-solving skills.
- Ability to work independently and manage a diverse patient caseload effectively.
- Comfort and proficiency with telehealth platforms, EHR systems, and virtual communication tools.
Preferred Qualifications
- Previous experience in a telehealth or remote patient care setting.
- Bilingual proficiency (especially Spanish) to serve a diverse patient population.
- Experience with various EMR/EHR systems.
- Certification in Advanced Cardiac Life Support (ACLS) or Basic Life Support (BLS).
