Key Responsibilities
- Conduct virtual health assessments and remote patient consultations via phone and video.
- Provide telephone triage and clinical advice for various acute and chronic health conditions.
- Develop and implement individualized remote care plans in collaboration with the medical team.
- Educate patients on chronic disease management, medication adherence, preventative health, and self-care through virtual platforms.
- Monitor patient progress remotely, identify changes in health status, and adjust care plans as necessary.
- Accurately document all patient interactions, assessments, and interventions in the electronic health record (EHR) system.
- Collaborate effectively with on-site clinical teams, medical practitioners, and other healthcare professionals to ensure continuity of care.
- Ensure compliance with all healthcare regulations, ethical standards, and company policies regarding patient privacy and care.
- Utilize telehealth technologies to deliver high-quality, patient-centered care and improve access to health services.
- Offer empathetic support and guidance to patients and their families, fostering a trusting virtual relationship.
Required Skills
- Current registration as a Registered Nurse with the South African Nursing Council (SANC).
- Minimum of 2 years post-registration clinical experience, preferably in a clinic or primary care setting.
- Demonstrated proficiency in clinical assessment, critical thinking, and decision-making.
- Excellent verbal and written communication skills for effective virtual interactions.
- Strong computer literacy and comfort with telehealth platforms, electronic health record (EHR) systems, and virtual communication tools.

Preferred Qualifications
