Job Description

We are seeking a passionate and dedicated Remote Community Health Worker (CHW) to join our dynamic team at Thaba Nchu Hospital. This crucial role involves working primarily from a remote setup, but with significant engagement directly within the communities of Thaba Nchu. As a Remote CHW, you will act as a vital link between the hospital’s services and the local population, promoting health, preventing disease, and facilitating access to care. Your work will involve conducting scheduled home visits, organizing community health education sessions (both in-person and virtually where appropriate), and utilizing digital tools for reporting and communication. You will empower individuals and families by providing essential health information, assisting them in navigating the healthcare system, and supporting them in adopting healthy behaviors. This role requires a high degree of autonomy, excellent communication skills, and a genuine commitment to improving public health outcomes in a community setting. While you will coordinate with the hospital team remotely for supervision, training, and team meetings, your primary operational base will be within the community, requiring you to manage your time effectively and be proactive in your outreach. You will be instrumental in identifying health needs, addressing social determinants of health, and fostering trust within the communities we serve.

Key Responsibilities

  • Conduct regular remote and in-person household visits within the Thaba Nchu community to assess health needs and provide health education.
  • Offer guidance and support on various health topics, including chronic disease management, maternal and child health, communicable diseases, nutrition, hygiene, and immunization.
  • Facilitate referrals to Thaba Nchu Hospital and other appropriate healthcare services, assisting community members with appointment scheduling and follow-ups.
  • Monitor and support adherence to treatment plans and health recommendations.
  • Maintain accurate and confidential digital records of all client interactions and community engagements, submitting reports remotely as required.
  • Participate in regular virtual team meetings, training sessions, and professional development opportunities.
  • Organize and lead community health promotion activities and workshops.
  • Act as an advocate for community members, articulating their needs and concerns to healthcare providers.
  • Collect and report data on community health indicators to inform public health strategies.

Required Skills

  • Proven ability to communicate effectively with diverse populations, demonstrating empathy and cultural sensitivity.
  • Strong organizational and time management skills to work independently and manage a caseload.
  • Basic digital literacy, including proficiency in using smartphones, tablets, and basic office software for reporting and communication.
  • Knowledge of common health issues and prevention strategies relevant to the Free State community.
  • Problem-solving abilities and a proactive approach to addressing community health challenges.
  • Ability to work effectively both independently and as part of a remote team.

Preferred Qualifications

  • A certificate or diploma in Community Health Work, Nursing, Social Work, or a related field.
  • At least 1 year of experience working as a Community Health Worker or in a similar community outreach role.
  • Valid South African driver's license and access to reliable personal transportation (essential for community visits).
  • Fluency in local languages such as Setswana or Sesotho, in addition to English.
  • Experience with public health campaigns or chronic disease management programs.
