Key Responsibilities
- Provide comprehensive remote medication therapy management (MTM) services, including medication reconciliation and adherence monitoring.
- Conduct virtual patient consultations via video or phone to assess medication needs, provide education, and address concerns.
- Verify and process electronic prescriptions, ensuring accuracy, legality, and clinical appropriateness in compliance with South African regulations.
- Offer expert drug information and clinical support to patients and other healthcare professionals remotely.
- Monitor patient drug regimens for efficacy, adverse effects, and potential drug-drug interactions.
- Document all patient interactions and interventions accurately and thoroughly within the designated telepharmacy system.
- Collaborate effectively with a virtual team of healthcare providers, including doctors and nurses, to optimize patient care plans.
- Develop and implement patient-specific care plans to achieve therapeutic goals and improve health outcomes.
- Participate in remote continuous professional development activities to stay current with pharmaceutical advancements and best practices.
- Adhere strictly to all ethical guidelines, professional standards, and regulatory requirements set by the South African Pharmacy Council (SAPC).
Required Skills
- Bachelor of Pharmacy (BPharm) degree or equivalent.
- Current registration with the South African Pharmacy Council (SAPC) and in good standing.
- Minimum 3 years of post-registration experience as a practicing Pharmacist.
- Proficiency in using telepharmacy platforms and other digital health technologies.
- Exceptional verbal and written communication skills for virtual patient and peer interaction.
- Strong clinical judgment and problem-solving abilities.
