Key Responsibilities
- Conduct virtual health assessments and consultations using telehealth platforms.
- Provide remote patient monitoring, follow-up care, and medication management support.
- Develop and implement individualized care plans based on patient needs and clinical guidelines.
- Educate patients and their families on health conditions, treatment plans, medication, and lifestyle modifications via virtual channels.
- Triage patient calls and inquiries, accurately assessing symptoms and determining appropriate levels of care or referral.
- Collaborate effectively with interdisciplinary healthcare teams (physicians, specialists, social workers) through secure digital communication.
- Maintain accurate, comprehensive, and confidential electronic health records (EHRs) in compliance with professional and regulatory standards.
- Participate actively in quality improvement initiatives aimed at enhancing remote care service delivery and patient outcomes.
- Adhere strictly to professional nursing standards, ethical guidelines, and patient confidentiality (POPIA compliance).
- Utilize telehealth technology and other digital tools effectively for patient engagement, data collection, and communication.
Required Skills
- Valid and current registration with the South African Nursing Council (SANC) as a Registered Nurse.
- Minimum of 3 years of proven clinical experience in a hospital or primary care setting.
- Exceptional verbal and written communication skills with a clear, empathetic, and professional tone.
- Proficiency in using electronic health record (EHR) systems and comfort with learning new telehealth platforms.
- Strong critical thinking, problem-solving, and decision-making abilities in a remote context.
- Ability to work independently, manage time effectively, and maintain high productivity in a home-based environment.
- High level of empathy, compassion, and a patient-centered approach to care.
- Reliable high-speed internet connection and a dedicated, private, and professional home workspace free from distractions.
Preferred Qualifications
- Bachelor of Nursing Science degree.
- Post-basic diploma or specialty qualification (e.g., Primary Healthcare, Critical Care, Case Management).
- Previous experience in telehealth, remote patient monitoring, or virtual case management roles.
- Certification in a specialty area (e.g., wound care, diabetes education, mental health).
- Fluency in an additional South African language relevant to the Limpopo region (e.g., Sepedi, Tsonga, Venda).
