Key Responsibilities
- Conduct virtual patient assessments, including symptom evaluation and health history review, through secure telehealth platforms.
- Provide comprehensive health education and counselling to patients and their families on disease management, medication adherence, and preventive care.
- Monitor and evaluate patient progress, particularly those with chronic conditions, utilising remote monitoring devices and electronic health records.
- Develop, implement, and review individualised care plans in collaboration with patients, their primary care physicians, and other healthcare professionals.
- Coordinate referrals to specialists or other healthcare services as needed, ensuring continuity of care.
- Respond to patient inquiries and provide timely, accurate medical advice and support.
- Maintain meticulous and confidential patient records, documenting all interactions and care provided.
- Participate in virtual team meetings and professional development opportunities to enhance clinical skills and knowledge.
- Adhere strictly to all ethical guidelines, professional standards, and company policies, ensuring patient safety and privacy.
Required Skills
- Current registration with the South African Nursing Council (SANC) as a Registered Nurse.
- Minimum of 2 years post-basic nursing experience in a clinical setting.
- Proven experience with electronic health records (EHR) systems.
- Excellent verbal and written communication skills in English (Afrikaans an advantage).
- Strong clinical assessment and critical thinking abilities.
- Proficiency in using digital communication tools and telehealth platforms.
- Ability to work autonomously and manage time effectively in a remote environment.
- Strong patient education and counselling skills.
- Demonstrated empathy, compassion, and patient-centred approach.
