Key Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized care plans for clients.
- Provide counseling, crisis intervention, and emotional support to individuals, families, and groups.
- Advocate for clients' rights and facilitate access to essential community resources, including healthcare, education, housing, and social services.
- Collaborate effectively with interdisciplinary teams, external agencies, government bodies, and community leaders.
- Maintain accurate, confidential, and timely case documentation in compliance with ethical and legal standards.
