Job Description

Family Health Centers of San Diego is seeking a dynamic and compassionate Health Educator to join our dedicated team in Downtown Chula Vista. In this crucial role, you will be instrumental in designing, implementing, and evaluating health education programs and services tailored to the diverse needs of our patient population and the wider community. You will serve as a vital resource, providing accurate, evidence-based health information on a wide array of topics, including chronic disease management, nutrition, physical activity, women’s health, preventive care, and substance abuse prevention. This position demands a creative approach to health promotion, utilizing various channels from one-on-one counseling and group workshops to community outreach events and the development of engaging educational materials. You will collaborate closely with medical providers, social workers, and other community partners to ensure a holistic approach to patient care and community wellness. Your work will directly contribute to improving health outcomes, reducing health disparities, and fostering a culture of wellness within the Chula Vista community. We are looking for an individual who is not only knowledgeable in health education principles but also possesses exceptional communication skills, cultural competency, and a genuine desire to make a lasting positive impact. Key Responsibilities

  • Develop, implement, and evaluate health education programs, workshops, and individual counseling sessions for diverse populations.
  • Provide evidence-based information on various health topics, including chronic disease prevention, healthy lifestyles, family planning, and substance abuse.
  • Create engaging and culturally appropriate health education materials (e.g., brochures, presentations, online content).
  • Conduct community outreach and participate in health fairs and events to promote FHCSD services and health awareness.
  • Collaborate with healthcare providers, social workers, and other community organizations to integrate health education into comprehensive care plans.
  • Track and report on program participation, outcomes, and effectiveness to ensure continuous improvement.
  • Serve as a patient advocate, connecting individuals with appropriate resources and support services.
  • Maintain up-to-date knowledge of health education trends, public health guidelines, and community health needs.

Required Skills

  • Bachelor's degree in Public Health, Health Education, or a related field.
  • 2+ years of experience in health education, community health, or a related role.
  • Strong understanding of health education theories, principles, and best practices.
  • Excellent verbal and written communication skills, with the ability to explain complex health information clearly and concisely.
  • Proven ability to conduct engaging presentations and facilitate group discussions.
  • Cultural competency and sensitivity to work effectively with diverse patient populations.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).

Preferred Qualifications

  • Master's degree in Public Health or Health Education.
  • Certified Health Education Specialist (CHES) or Master Certified Health Education Specialist (MCHES) certification.
  • Bilingual proficiency (English/Spanish) highly preferred.
  • Experience working in a community health center or Federally Qualified Health Center (FQHC) environment.
  • Familiarity with the specific health needs and resources within the Chula Vista community.
