Job Description

LifeSkills Inc. is actively seeking a highly motivated, empathetic, and dedicated Full-Time Case Manager to become a vital member of our collaborative team in Bowling Green, Kentucky. In this essential role, you will be the driving force behind coordinating and advocating for individuals grappling with mental health conditions, substance use disorders, or developmental disabilities. Your primary mission will be to ensure these individuals receive access to comprehensive, person-centered, and fully integrated care tailored to their unique circumstances. As a Case Manager, you will serve as a crucial central point of contact, expertly guiding clients through often complex and fragmented service systems. This involves meticulously developing individualized service plans, diligently monitoring their progress towards clearly defined goals, and navigating the intricacies of local and state resources. This position demands a profound commitment to client advocacy, exceptional organizational acumen, and the proven ability to work seamlessly within a multidisciplinary team. You will be instrumental in empowering clients to achieve greater independence, foster improved well-being, and enhance their overall quality of life by connecting them with vital community resources, facilitating timely access to critical medical and therapeutic services, and providing ongoing, unwavering support and education. Your impactful work will directly contribute to the betterment of individuals and families, instilling hope and actively promoting recovery and thriving within our beloved community.

Key Responsibilities

  • Conduct comprehensive assessments to identify client needs, strengths, and goals.
  • Develop and implement individualized service plans (ISPs) in collaboration with clients, families, and treatment teams.
  • Coordinate and link clients to appropriate community resources, including medical, mental health, substance use, housing, employment, and social services.
  • Provide ongoing advocacy to ensure clients' rights are protected and their needs are met.
  • Monitor client progress, regularly reviewing and adjusting ISPs as needed to reflect changing circumstances.
  • Maintain accurate and timely documentation of all client interactions, services, and outcomes in accordance with organizational and regulatory standards.
  • Collaborate effectively with interdisciplinary teams, external agencies, and family members to ensure integrated care.
  • Educate clients and families on available resources, service options, and self-advocacy skills.
  • Participate in regular supervision, team meetings, and professional development activities.
  • Respond to crisis situations as needed, providing support and facilitating appropriate interventions.

Required Skills

  • Excellent written and verbal communication skills
  • Strong organizational and time management abilities
  • Proficient in crisis intervention and de-escalation techniques
  • Ability to empathize with and effectively relate to diverse client populations
  • Competency in using electronic health records (EHR) systems and Microsoft Office Suite
  • Knowledge of community resources in Bowling Green and South Central Kentucky
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
  • Valid Kentucky Driver's License and reliable transportation

Preferred Qualifications

  • Bachelor's degree in Social Work, Psychology, Counseling, or a related human services field
  • Previous experience in case management, social work, or a related field within a behavioral health or developmental disabilities setting
  • Understanding of state and federal regulations pertaining to mental health and developmental disability services
  • Bilingual proficiency (e.g., Spanish, Arabic)
