Key Responsibilities
- Conduct comprehensive virtual patient assessments, including history taking, physical examination (via telehealth tools), and psychosocial evaluations.
- Diagnose and treat common acute and chronic illnesses, injuries, and health conditions using evidence-based practice guidelines.
- Order, interpret, and evaluate diagnostic tests such as lab work and imaging studies.
- Develop and implement individualized patient care plans, including medication management and non-pharmacological interventions.
- Provide health education, counseling, and preventative care services to promote patient wellness and self-management.
- Refer patients to specialists or other healthcare providers as necessary, facilitating seamless coordination of care.
- Maintain accurate, timely, and thorough electronic health records (EHR) documentation.
- Adhere to all state and federal regulations, professional ethics, and company policies related to telehealth practice.
- Participate in quality improvement initiatives and continuing education to enhance clinical skills and knowledge.
- Collaborate effectively with interdisciplinary healthcare teams to optimize patient outcomes.
Required Skills
- Current and unrestricted Indiana Registered Nurse (RN) license.
- Current and unrestricted Indiana Nurse Practitioner (NP) license with prescriptive authority.
- Master's or Doctorate degree in Nursing from an accredited program.
- Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or Pediatric Nurse Practitioner (PNP).
- Minimum of 2 years of experience as a practicing Nurse Practitioner.
- Proficiency in using Electronic Health Record (EHR) systems and telehealth platforms.
- Excellent verbal and written communication skills.
