Key Responsibilities
- Conduct comprehensive virtual health assessments, including history taking, symptom review, and physical examination (as appropriate via video).
- Diagnose and treat common acute and chronic illnesses, injuries, and health conditions within the scope of practice.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient conditions.
- Develop and implement individualized patient care plans, including medication management and lifestyle recommendations.
- Prescribe medications, treatments, and therapies in accordance with state and federal prescribing regulations.
- Provide patient education and counseling on health maintenance, disease prevention, and treatment adherence.
- Maintain accurate and timely electronic health records (EHRs) for all patient encounters.
- Collaborate with other healthcare professionals as needed to ensure coordinated patient care.
- Participate in quality improvement initiatives and continuing education to enhance clinical skills and knowledge.
- Adhere to all HIPAA regulations and maintain patient confidentiality.
- Manage patient follow-ups, referrals, and coordinate care transitions effectively.
Required Skills
- Active, unrestricted Nurse Practitioner license in Texas.
- Master's or Doctorate Degree in Nursing (MSN/DNP) from an accredited program.
- Board Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (AGNP), or Pediatric Nurse Practitioner (PNP).
- DEA registration.
- Minimum of 2 years of experience as a practicing Nurse Practitioner.
- Demonstrated proficiency in utilizing telehealth platforms and electronic health records (EHR) systems.
- Excellent verbal and written communication skills.
- Strong diagnostic and critical thinking abilities.
- Ability to work independently and manage a diverse patient caseload effectively.
- Commitment to patient-centered care and cultural competency.
Preferred Qualifications
- Previous experience specifically in a telehealth or remote patient care setting.
