This is an exciting opportunity to join a dynamic multidisciplinary team focused on delivering high-quality, patient-centred care within a community setting. As an Occupational Therapist, you will play a pivotal role in empowering individuals to achieve their maximum level of independence and enhance their overall quality of life.
In this role, you will be responsible for assessing, planning, implementing, and evaluating therapeutic interventions for a diverse caseload of patients with a variety of physical, mental, and social health needs. You will work closely with patients, their families, carers, and other healthcare professionals to develop personalized treatment plans that address functional limitations, promote self-management, and facilitate participation in meaningful daily activities. Your expertise will be crucial in prescribing equipment, adapting environments, and teaching compensatory strategies to enable individuals to overcome challenges and achieve their personal goals. Key Responsibilities
- Conduct comprehensive occupational therapy assessments, interpreting findings to identify an individual's strengths, needs, and goals.
- Develop and implement evidence-based, patient-centred intervention plans to address functional deficits and promote independence.
- Provide therapeutic interventions, including activity analysis, rehabilitation exercises, and functional retraining.
- Prescribe, issue, and train patients/carers in the use of assistive technology, adaptive equipment, and home modifications.
- Evaluate the effectiveness of interventions, modifying treatment plans as necessary to achieve desired outcomes.
- Maintain accurate and timely patient records in accordance with professional and organizational standards.
- Work collaboratively as part of a multidisciplinary team, contributing to case conferences and care planning.
- Provide education and support to patients, families, and carers regarding their condition and self-management strategies.
- Participate in service development, quality improvement initiatives, and continuous professional development.
- Adhere to all relevant professional codes of conduct, local policies, and national guidelines.
Required Skills
- BSc or MSc in Occupational Therapy
- HCPC registration (Health and Care Professions Council)
- Minimum of 2 years post-qualification experience as an Occupational Therapist
- Strong assessment, analytical, and problem-solving skills
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals
- Proven ability to manage a varied caseload independently and effectively
- Knowledge of relevant legislation and best practice in occupational therapy
- Full UK driving licence and access to a vehicle for community visits
Preferred Qualifications
- Experience working within a community health setting
- Specialist skills in areas such as neuro-rehabilitation, complex seating, or manual handling
- Experience supervising junior staff or students
- Participation in continuous professional development relevant to the role
