Key Responsibilities
- Develop and manage personalised care plans in collaboration with clients, families, and healthcare professionals.
- Coordinate and schedule care visits, ensuring continuity and appropriate staffing levels.
- Act as the primary point of contact for clients, families, and carers, addressing enquiries and concerns promptly.
- Conduct initial assessments and regular reviews of care packages, making adjustments as necessary.
- Ensure compliance with all regulatory standards, company policies, and best practices in care delivery.
- Maintain accurate and up-to-date client records, utilising relevant IT systems.
- Provide support and guidance to care staff, ensuring they are equipped to deliver high-quality care.
- Participate in on-call rotations as required to provide out-of-hours support.
Required Skills
- Proven experience in care coordination or a similar role within a care setting (minimum 2 years).
- Excellent organisational and time management abilities.
- Strong interpersonal and communication skills (written and verbal).
- Proficiency in using care management software and general office applications.
- Ability to work independently and as part of a team.
- High level of empathy, patience, and compassion.
- Full UK driving license and access to a reliable vehicle.
Preferred Qualifications
- NVQ Level 3 in Health and Social Care (or equivalent).
- Knowledge of local health and social care services in Kent.
- Experience with CQC regulations and compliance.
- First Aid certification.
