Key Responsibilities
- Conduct comprehensive psychosocial assessments to determine patient needs and develop appropriate treatment plans.
- Provide individual, group, and family counseling utilizing evidence-based therapeutic techniques.
- Develop and implement individualized treatment plans, setting measurable goals and objectives.
- Monitor patient progress, adjust treatment plans as necessary, and document all clinical activities accurately and promptly.
- Collaborate with a multidisciplinary team including physicians, nurses, and social workers to ensure integrated patient care.
- Educate patients and families on mental health conditions, coping strategies, and available community resources.
- Facilitate referrals to external services or higher levels of care when appropriate.
- Maintain strict confidentiality and adhere to all ethical and professional standards.
- Participate in case conferences, supervision, and ongoing professional development activities.
Required Skills
- Master's degree in Counseling, Social Work, Psychology, or related field.
- Active and unrestricted license as a Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or equivalent in the State of Delaware.
- Minimum of 2 years of post-licensure clinical experience in mental health counseling.
- Proficiency in conducting diagnostic assessments and developing individualized treatment plans.
