Job Description

We are seeking a highly motivated and patient-focused Clinical Pharmacist to join our esteemed team at Detroit Receiving Hospital. In this pivotal role, you will be instrumental in optimizing medication therapy for our diverse patient population across various clinical settings. Your expertise will directly impact patient outcomes by ensuring the safe, effective, and rational use of medications. This position offers a unique opportunity to apply advanced pharmaceutical knowledge in a high-acuity environment, collaborating closely with physicians, nurses, and other healthcare professionals. You will be expected to provide comprehensive pharmaceutical care, participate in interdisciplinary rounds, and contribute to medication management initiatives. Key Responsibilities

  • Provide comprehensive pharmaceutical care, including medication order review, verification, and dispensing, ensuring accuracy and appropriateness.
  • Actively participate in interdisciplinary patient care rounds, offering expert consultation on pharmacotherapy, drug selection, dosing, and administration.
  • Monitor patient medication profiles for drug-drug interactions, contraindications, allergies, and adverse drug reactions, intervening as necessary.
  • Conduct medication reconciliation upon admission, transfer, and discharge to prevent medication errors and ensure continuity of care.
  • Educate patients and healthcare professionals on proper medication use, potential side effects, and adherence strategies.
  • Participate in formulary management, medication utilization reviews, and quality improvement initiatives.
  • Supervise pharmacy technicians and ensure compliance with all federal, state, and local regulations, as well as hospital policies and procedures.
  • Contribute to the development and implementation of clinical protocols and guidelines to enhance patient safety and optimize drug therapy.
  • Maintain accurate and complete documentation of all pharmaceutical care activities.
  • Participate in staff education, preceptorship, and continuous professional development activities.

Required Skills

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
  • Current, unrestricted Pharmacist licensure in the State of Michigan.
  • Minimum of 2 years of acute care hospital pharmacy experience.
  • Demonstrated ability to perform clinical pharmacy interventions and provide drug information.
  • Proficiency in electronic health record (EHR) systems and pharmacy management software.
  • Strong understanding of pharmacokinetics, pharmacodynamics, and medication safety principles.
  • Exceptional verbal and written communication skills.
  • Ability to work effectively in a fast-paced, high-pressure environment.

Preferred Qualifications

  • Completion of an ASHP-accredited PGY1 residency program.
  • Board Certification in Pharmacotherapy (BCPS) or other relevant specialties.
  • Experience in specialized areas such as critical care, emergency medicine, or infectious diseases.
  • Demonstrated leadership potential or experience mentoring junior staff.
